‘Transforming Your Care’ Involving you in local changes in health and social care
The public are invited to attend local engagement meetings to discuss ‘Transforming Your Care’: A Review of Health and Social Care in Northern Ireland and the development of a Local Population Plan and how it will be taken forward in the Southern area.
The Southern Local Commissioning Group and Southern Health and Social Care Trust have been asked to jointly prepare a Local Economy Population Plan for the Southern area by end of June 2012. ‘Population Plans’ are plans for the next three years and beyond, and set out proposals for how we can address the growing needs and expectations of our local population and provide safe, personal, effective, affordable care into the future.
